List of Nominee's Major Career and Business Accomplishments

Elaborate on how the nominee has demonstrated leadership, innovation and initiative on the job (e.g., job maintenance or creation, training, new approaches, personal growth, leadership, coaching, enhanced company profits, customer service). Indicate how the employee has contributed to the success of the business and his/her career and professional excellence.

Describe the ways the nominee has exhibited leadership with active commitment to the improvement of the quality of life within Somerset County in recent years. This criteria is evaluated, but given lesser importance than the nominee's business accomplishments.

NOTE:  We will also need a signed letter from the nominee verifying the accuracy of the nomination form, accepting the nomination and agreeing to be present at the awards luncheon.
